Youth Programs Terms and Conditions
I agree for my child to participate, and/or agree to allow my child to participate in Gage Academy of Art art program, which includes, but is not limited to, working with various art supplies, time in the onsite playground as well as possible walking trips to Volunteer park to complete assignments.
1. Notice and Representation
All Programs contain inherent unforeseen risks that cannot be anticipated, controlled or eliminated by Gage Academy of Art., its agents and representatives have no duty to do so. You represent, certify, warrant, and affirm that - in each instance your child has the minimum physical fitness necessary to participate in the Program without injury to themselves or others, (b) are then in a healthy medical and physical state, are not impaired in any way, and (c) understand that your student has the right to not participate in any activity you or your child wishes not to participate.
2. Assumption of Risk
I understand and agree that by participating you and your child voluntarily engage in and assume the risk of any physical activity, which could result in property damage and/or bodily injury.
For good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, you, your heirs, assigns, agents, and representatives hereby expressly agree to defend, indemnify and hold harmless Gage Academy of Art, its directors, officers, shareholders, employees, agents, representatives, and any of their successors or assigns from and against Loss and all suits, actions, claims, demands, loss or damages (including reasonable attorney's fees) that arise from, are related to, or in connection with your student’s participation in a Program.
4. Waiver and Release
You do hereby release Gage Academy of Art, its directors, officers, shareholders, employees, agents, representatives, and any of their successors or assigns of and from Loss and any liability whatsoever, which you may sustain or suffer that arises from, is related to, or in connection with (a) your child’s participation in a Program.
5. Photographs and Recordings
Gage Academy of Art, teens and children may be recorded and photographed for marketing, website, social media, archival and/or other purposes. Such recording or photographing may include pictures of the attendees, individuals at our program or class participants. Your, your child and/or guest attendance at any class or event at Gage Academy of Art shall be deemed your consent to appear in such programming and/or photographs without compensation or additional approval of any kind.
6. Drop Off & Pick Up
a) Child under 13: I will sign my child in and out from Gage Programs daily.
b) Child over 13: I understand my teen will sign themselves in and out and may not leave campus between the hours of 9:30 am through 3:30 pm, including lunch period.
c) I will drop off/pick up my child not more than 10 minutes before or after the program, otherwise I understand that my child will become part of Gage’s Creative Care program for which I will automatically be billed an additional $1/minute.
7. Behavior & Dress Code
a) I understand that if my child exhibits aggressive or disruptive behavior, Gage Academy of Art staff has the right to remove your child from the program without refund.
b) There is a no smoking, no alcohol and no illegal substance policy. This policy is strictly enforced.
c) Your child will need to wear clothes that can be dirtied and messy.
d) Your child will need to wear closed toed shoes.
8. Consent for Treatment
a) I consent and authorize Gage Academy of Art to provide treatment whether en route, on or off campus for any first aid whether routine or emergency, including, without limitation, injury, illness, choking, etc.
b) I consent and authorize Gage Academy of Art instructors, staff and/or first responders, to provide treatment, including cardiopulmonary resuscitation (CPR) if needed.
c) If you, or assigned guardians, cannot be reached in case of an emergency or other medical need, I hereby appoint, authorize and constitutes Gage Academy of Art staff, nurse, doctor, first responder or any other qualified individual to act on behalf as parents to authorize and consent to medical treatment for child.
d) I consent that my child may be transported off the Gage Academy of Art campus by qualified first responders for any treatments as noted above.
e) I certify that my child is covered by medical insurance for the duration of the Program for which they are registered.
f) If child arrives ill or becomes ill during the program, I understand that they will be required to be removed from the program and picked up by myself or assigned guardian.
g) I understand that Gage Academy of Art will not administer any drugs to my child, whether prescription or over-the-counter.
